Pathways Hospice

Located in Fort Collins since 1978, Pathways provides care for those navigating the last months of life and support programs for their families. Pathways serves individuals living in Larimer and Weld Counties, including Fort Collins, Loveland, Greeley and Windsor.

Pikes Peak Hospice & Palliative Care introduced the hospice movement to the Pikes Peak region in 1980. Since that time, Pikes Peak Hospice teams have continued to offer compassionate, expert care and practical support to ensure comfort for all who face serious illness.
