• Assist individuals and families affected by Parkinson’s Disease cope with their emotional and physical needs. From first diagnosis, change in health status, crisis and advanced stages of Parkinson’s. This position will be in office 5 days per week (some weekends required for special events).
Under general professional supervision, performs behavioral health case coordination services in the community including planning, organizing, and treatment referral/disposition. Engages patients through community outreach, both independently and with various external partners, to assess patient needs and develop plan for accessing care.
